Pathways to Practice is expanding its network of Community Health Workers (CHWs) and Birth Doulas throughout California and is seeking compassionate, community-minded individuals who are passionate about improving health outcomes and supporting underserved populations.

As an Independent Contractor, you will provide non-clinical support, health education, care navigation, and community-based services to individuals and families across California. Our programs serve diverse populations, including pregnant and postpartum individuals, Medi-Cal members, justice-impacted individuals, individuals returning from incarceration, and communities experiencing health and social disparities.

Key Responsibilities
- Provide health education, outreach, and resource navigation services.
- Support pregnant and postpartum individuals through culturally responsive doula services.
- Connect clients to healthcare providers, social services, and community-based resources.
- Conduct community outreach and engagement activities.
- Assist individuals in overcoming barriers related to healthcare access, transportation, housing, food insecurity, and other social determinants of health.
- Educate clients about available benefits, wellness programs, and community resources.
- Maintain accurate and timely documentation using secure electronic systems.
- Collaborate with healthcare organizations, managed care plans, community-based organizations, and support networks.
- Participate in onboarding, professional development, and required training programs.
- Adhere to HIPAA, confidentiality, and organizational compliance requirements.

Preferred Qualifications
- Community Health Worker certification (preferred but not required)
- Birth Doula certification (preferred but not required)
- Experience working with underserved, vulnerable, or marginalized populations
- Experience supporting pregnant, postpartum, or parenting individuals
- Experience working with Medi-Cal populations
- Background in outreach, advocacy, case management, care coordination, peer support, or health education
- Experience supporting justice-impacted or reentry populations
- Strong interpersonal, communication, and relationship-building skills
- Ability to work independently while maintaining professionalism and accountability
- Bilingual or multilingual candidates are strongly encouraged to apply
