Substance Use and Co-Occurring Disorders | Partial Hospitalization Program (PHP)
Real Recovery Solutions is seeking a full-time PHP Primary Therapist for our Bradenton location. The PHP Primary Therapist serves as the primary clinical point of contact for an assigned caseload, delivering evidence-based, trauma-informed individual, family, and group therapy to adults in substance use disorder and co-occurring treatment. Reporting to the Clinical Director, with day-to-day clinical supervision from the assigned Clinical Lead or Qualified Supervisor, the PHP Counselor guides clients through the treatment process while maintaining a clinical record that demonstrates medical necessity and supports payor and surveyor review.
Duties and Responsibilities
- Serve as the primary clinical contact for an assigned PHP caseload, providing individual, family, and group therapy.
- Provide 25 direct client contact hours per week, with an average of 15 of those hours delivered in the group therapy setting.
- Provide individual therapy at least weekly, and family or couples sessions as indicated by the treatment plan.
- Facilitate psychoeducational and process groups using the RRS group curriculum and SAMHSA TIP 41 modalities, with group size not exceeding 15.
- Complete the Bio-Psychosocial Assessment, trauma screen, and suicide risk assessment (SAFE-T with C-SSRS), along with the ASAM admission, within required timeframes.
- Develop individualized Master Treatment Plans, signed by client and counselor, and complete treatment plan reviews at least every 30 days.
- Conduct ongoing reassessment and level-of-care transition evaluations; gather assessment data supporting the LPHA diagnostic determination and obtain LPHA review and co-signature of the treatment plan.
- Document all services in KIPU in DAP format within the 24 business-hour standard, ensuring each note is individualized, medically necessary, and payor-defensible.
- Complete concurrent and utilization reviews that support continuing medical necessity, and coordinate care with medical, nursing, case management, and peer support staff.
- Develop and implement discharge and continuing-care plans, and close out treatment plans and reviews at discharge.
- Conduct ongoing suicide and self-harm risk assessment and safety planning; respond to and de-escalate behavioral crises in a trauma-informed, strengths-based manner; and initiate Baker Act and Marchman Act processes when clinically and legally indicated.
- Attend the daily clinical and operational update, treatment team, and interdisciplinary team meetings, and participate in weekly group clinical supervision.
- Protect client privacy and comply with HIPAA, 42 CFR Part 2, Florida DCF Rule 65D-30, and The Joint Commission standards.
Required Skills and Abilities
- Working knowledge of Florida DCF 65D-30, the ASAM Criteria (Fourth Edition), DSM-5-TR, TJC Behavioral Health Care standards, HIPAA, and 42 CFR Part 2.
- Familiarity with evidence-based SUD modalities, including Motivational Interviewing, CBT, DBT, TF-CBT, and 12-Step facilitation, and with ASAM patient placement criteria.
- Proficiency in DAP documentation that supports medical necessity and utilization review.
- Strong organizational, communication, problem-solving, and crisis-response skills, with the ability to manage a full caseload and consistently meet documentation deadlines.
- Comfort working with electronic medical records and clinical technology (KIPU and ClinicalNotes.ai experience strongly preferred; Microsoft Office, Slack, and Gmail).
Education and Experience
- Master's degree in Counseling, Social Work, Marriage and Family Therapy, Psychology, or a closely related behavioral health field (required).
- Active Florida license (LMHC, LCSW, LMFT, or Licensed Psychologist) strongly preferred.
- Registered interns (RMHCI, RCSWI, RMFTI) are considered, practicing under qualified supervision.
- Certified Addiction Professionals (CAP or MCAP) are considered.
- CPR and First Aid certification required within 30 days of hire.
- Minimum of 1 year of clinical experience in a substance use, behavioral health, or co-occurring disorders setting preferred; practicum and internship experience recognized.
- Trauma-focused certification (CCTP, EMDR, or CPP) preferred but not required.
Schedule
- First Shift: Monday to Friday, 9:00 AM to 5:00 PM.

Pay: $55,000.00 - $70,000.00 per year
