Kettering Home Care is seeking a dedicated Physical Therapist (PT) to deliver high-quality, home-based therapy. As a vital member of our clinical team, the Physical Therapist (PT) will complete comprehensive initial evaluations, develop custom plans of care, and guide patients through their recovery journey. The Physical Therapist (PT) ensures clinical excellence, supervises assisting staff, and provides essential patient and family education. Territory: Dayton/West Carrollton (45439) and Kettering (45429)

Requirements
- Current license as a Physical Therapist (PT) in the state of employment.
- Minimum of one year of experience as a PT in an acute care setting required (Home care experience preferred).
- Valid driver’s license and active auto insurance listing your name as a covered driver.
- Proficiency with OASIS assessments and a strong understanding of Medicare/Federal guidelines.
- Ability to lift up to 50 lbs. frequently and meet healthcare health screenings (negative TB test).
Responsibilities & Essential Functions
- Assessments & Care Plans: Perform comprehensive OASIS assessments, complete initial evaluations within 48–72 hours of referral, and establish individualized clinical goals.
- Clinical Care: Provide skilled physical therapy interventions (exercise, massage, heat/water) to relieve pain, restore functional independence, and minimize re-hospitalization risks.
- Supervision & Collaboration: Oversee and supervise therapy services provided by PTAs and home health aides; communicate updates effectively across the care team.
- Patient Education: Counsel patients and families on disease/injury management, safety measures, and proper medical equipment usage.
- Documentation Timelines: Complete all clinical charting within company requirements (within 24 hours for routine/discharge/recertification visits; within 48 hours for start of care).
