Role Summary
The LPC/LCSW will provide targeted case management services to individuals and families needing support with mental health, social stability, and resource coordination. This role blends clinical insight with hands‑on case management, ensuring clients receive the services, referrals, and advocacy needed to improve functioning and quality of life. The clinician will work collaboratively with internal staff, community partners, and service providers to deliver high‑impact, person‑centered care.
Key Responsibilities
- Conduct initial assessments, psychosocial evaluations, and ongoing needs assessments to determine client eligibility and service needs.
- Develop individualized service plans (ISPs) with measurable goals aligned to TCM requirements.
- Provide targeted case management, including resource coordination, referrals, advocacy, and follow‑up to ensure service linkage and continuity.
- Maintain consistent client contact, including home visits, community visits, telehealth check‑ins, and crisis support as needed.
- Collaborate with community agencies, medical providers, mental health professionals, schools, and social service organizations.
- Document all services in compliance with Medicaid, state, and agency standards, ensuring accuracy, timeliness, and audit‑readiness.
- Monitor client progress, adjust service plans, and provide ongoing support to reduce barriers and improve outcomes.
- Participate in team meetings, case reviews, and professional development activities.
- Uphold ethical standards, confidentiality, and professional boundaries at all times.
Required Qualifications
- Active Texas LPC, LPC‑Associate (with supervision), LCSW, or LMSW (with supervision).
- Experience in mental health, case management, social services, or community support roles.
- Strong understanding of Medicaid TCM guidelines, psychosocial needs, and community resources.
- Ability to work independently, manage a caseload, and maintain organized documentation.
- Valid driver’s license and reliable transportation for community‑based work.
Preferred Qualifications
- Experience working with high‑needs populations (youth, adults, families, crisis cases).
- Familiarity with trauma‑informed care, motivational interviewing, and strengths‑based approaches.
- Bilingual (English/Spanish) is a plus.
