The activity Designee is appointed by the Med Surg Manager, to provide an activity program appropriate to the needs and interests of the patients. The designee reports to the Med Surg Manager.

Meets with the patient and family upon admission to the Swing Bed Program for review of social activities, lifestyle capabilities and deficits in mobility.

Responsible for making the patient aware of activity schedule, and placing emphasis on those activities which might be of special interest.

Involved in developing an overall Plan of Care for each patient in the Swing Bed Program, is aware of the needs according to physician orders, and problems anticipated.

Prepares monthly summary of activities of each patient’s multi-disciplinary progress note in the Swing Bed Program, which may be updated after evaluation.

Meets the individual needs of each patient by facilitating patient participation in recreational, diversional, religious, social, intellectual and (upon physician’s orders) work therapy activities.

Coordinates activity with nursing staff, taking into account established routines to include but not limited to meal assistance as needed, daily hygiene and physical therapy demands

Responsible for other tasks as designated by the Case Manager RN in accordance with quality and regulatory requirements.
