Dry Harbor Nursing & Rehabilitation Center, a 360-bed skilled nursing facility dedicated to delivering exceptional care to our patients and residents, is seeking a highly skilled and compassionate Nursing Concierge to join our team. Responsibilities:
- Greet and register patients, families, and visitors, while maintaining a welcoming and professional atmosphere
- Process and coordinate admissions, discharges, and transfers, ensuring accurate and timely documentation
- Provide emotional support and guidance to residents and families, addressing their concerns and needs
- Collaborate with the healthcare team to ensure a comprehensive care plan is in place, addressing the unique needs of each resident
- Conduct initial assessments and identify potential issues, escalating concerns to the nurse manager or other appropriate personnel as needed
- Maintain accurate and detailed records, adhering to industry standards and regulations
- Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures
Requirements:
Minimum of 3 years of experience in a healthcare setting
Strong clinical knowledge and nursing skills
Excellent communication and interpersonal skills
Ability to work in a fast-paced environment with multiple priorities
