Job Overview
We are seeking a dedicated and energetic Physical Therapist who is a Certified Lymphedema Therapist (CLT) or wants to become a Certified Lymphedema Therapist (CLT) to join our dynamic healthcare team. In this vital role, you will deliver personalized rehabilitation services to patients across various settings, including outpatient clinics, hospitals, inpatient units, and home health environments. Your expertise will help patients regain mobility, reduce pain, and improve their overall quality of life through tailored treatment plans. This position offers an exciting opportunity to work with diverse patient populations, including geriatrics, pediatrics, neurology, orthopedics, and more. You will collaborate closely with healthcare professionals to develop effective care strategies and ensure comprehensive patient care.
Responsibilities
- Assess patients’ physical conditions by conducting thorough evaluations of mobility, strength, flexibility, and functional ability.
- Develop and implement individualized treatment plans aligned with clinical best practices and patient goals.
- Deliver evidence-based physiotherapy interventions such as manual therapy, trigger point therapy, therapeutic exercises, and mobility training.
- Monitor patient progress regularly, adjusting treatment plans as needed to optimize recovery outcomes.
- Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
- Collaborate with multidisciplinary healthcare teams—including physicians, nurses, occupational therapists, and social workers—to coordinate comprehensive care.
- Educate patients and their families on treatment routines, discharge planning, home exercises, and community resources to support ongoing recovery.
- Manage discharge planning processes by preparing detailed care plans for patients transitioning from inpatient to outpatient or home settings.
- Work with diverse patient populations such as geriatric patients in skilled nursing facilities or pediatric patients in outpatient clinics.
- Ensure compliance with healthcare regulations and safety protocols during all patient interactions.
Requirements
- Valid state licensure or certification as a licensed Physical Therapist.
- Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, inpatient units or home health settings.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards.
- Experience working with geriatric patients and familiarity with geriatrics-specific care approaches.
- Demonstrated ability to develop effective treatment plans for diverse conditions including orthopedics and neurology cases.
- Proficiency in electronic health records (EHR) management systems or electronic medical records (EMR) systems.
- Excellent client communication skills to educate and motivate patients throughout their rehabilitation journey.
- Ability to work collaboratively with healthcare professionals across disciplines to deliver holistic patient care.
- Prior experience in outpatient rehabilitation centers or inpatient hospital environments is highly desirable.
- Knowledge of discharge planning procedures and home & community care practices is a plus.

Pay: $80,000.00 per year
