We are committed to bringing high-quality, relationship-based primary care directly to residents in a senior living community. As a Licensed Practical Nurse (LPN), you will partner closely with a Nurse Practitioner to provide compassionate, patient-centered care while supporting both clinical and administrative clinic functions.
This is a unique opportunity to help implement and shape an evolving care model in partnership with our Post-Acute Medical Group. Working in both an onsite clinic setting and resident living spaces, you will have greater autonomy than a traditional clinic-based LPN role while remaining connected to a collaborative care team. We are seeking someone who enjoys building meaningful relationships with older adults, takes initiative, embraces change, and is excited to contribute to the growth and success of this innovative program.
What you can expect from this role:
Schedule: 0.8 FTE (64 hours per pay period), Monday through Friday. Schedule is subject to change based on patient and clinic needs.
Responsibilities:

Partner with a Nurse Practitioner to deliver comprehensive primary care services, including preventive care, chronic condition management, follow-up visits, and care coordination.

Provide direct patient care within the LPN scope of practice, including collecting health information, performing clinical procedures, documenting care, and supporting medication reconciliation and care plan follow-up.

Support clinic operations and administrative functions, including patient scheduling, referral coordination, documentation, phone calls, and other activities that contribute to efficient clinic workflows (approximately 20% of role responsibilities).

Build trusted relationships with residents, families, community staff, and care team members while providing education and support to help patients navigate their healthcare journey.

Help develop and refine clinic workflows, processes, and operational standards while supporting implementation of this growing care model.
Qualifications:
Experience
-
Prior LPN experience preferred but not required.

New graduates are encouraged to apply. Candidates should have previous healthcare experience in a clinical setting, such as Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA), or a similar direct patient care role.
Education

Graduation from a Licensed Practical Nursing Program
Certifications

Current BLS-CPR Certification from the American Safety and Health Institute, American Heart Association or American Red Cross

Current Licensed Practical Nuse (LPN) licensure in the State of practice
