We offer services that provide persons with assistance and support needed to increase or maintain an individual's capacity for independent functioning, self-determination, interdependence, productivity and integration in the community. The services encourage, counsel, support, and assist individuals to maintain a safe and healthy living environment; to maintain good health and nutrition, including nutritional counseling; and to access the community, including finding and maintaining meaningful employment. Programmed activities are designed to increase or maintain the individual's skills in the areas of self-help, socialization, communication, leisure/recreational and adaptive skills for daily living.

Under the General Supervision of the Director, The Director of Nursing will perform the following duties:
DOH Requirements:
· Creating, complete and train Direct Support Staff on the following required medical participant documents when applicable:
1) Health Assessment Tool (HAT)
2) Medication Administration Assessment Tool (MAAT)
3) Health Care Plans (HCP)
4) Medical Emergency Response Plan (MERP)
5) Healthcare Passports
6) Therap Services
7) Aspiration Risk Screening Tool (ARST)
8) Medication Administration Records (MAR)
9) Nursing Quarterly Report
· Maintain all DDSD Adult Nursing related documentation in accordance with the current DDSD Standards
Agency Requirements:
· Complete nursing notes for all agency participants to include vitals, relative medical interview information and continuous tracking of all participant appointments
· Ensure all Direct Support Staff are in compliance with participants’ Physician and Psychiatrist orders
· Monitoring of all participants’ prescriptions
· Assisting/Answering medical related questions from staff and participants
· Compliance with all ABWL policy and procedures
· Assist/Attend/Coordinate Pharmacist Consultant Medication review for all programs
· QUICKMAR review with staff as needed
· Attending Quality Assurance Meetings and providing appropriate reports in assigned areas of monitoring
· Maintain positive relationships with all IDT members of each ABWL served participant
· Attending IDT meetings as assigned by management
· Participate in the new admission and discharge process for each participant
· Attending other participant meetings as assigned by ABWL
· Participate in Medical On-Call
· Incident Report Follow-up on participant medical related issues
· Other duties as required by DDSD Standards and/or ABWL
Nurse Qualifications:
· A minimum of a Bachelor’s Degree and Certified in the state of New Mexico as a Licensed Registered Nurse for at least a period of (1) calendar year.
· Minimum age of 21 years old
· Supervisory experience preferred.
· Cultural competence including but not limited to sensitivity to cultural variables in delivery of services needs.
· Valid N.M. Drivers license
· The ability to function as a Medical Liaison between Medical Professionals and ABWL Supportive Living
· Proficiency in managing medical situations regarding participants with Developmental Disabilities.
· Ability to work with others in a supportive, non-judgmental manner in the home and community environment.
· An understanding of the special problems related to the participants emotional, cognitive, behavioral, social, psychological dysfunctions and medical needs. The ability to create a safe and positive therapeutic milieu as a significant component of the participant’s ISP (Meaning, Purpose and Identity)
· Personal characteristics suitable for working with participants with special needs and the ability to consistently establish positive relationships communicate effectively and work cooperatively with participants, Colleagues and community members. The ability to teach others positive relationships building, communication and individual participant’s behaviors management skills.
· Ability to physically perform job duties
· Ability to work as a team member
· Ability to meet and comply with OSL, DDSD, Developmental Disabled and Licensing requirements at all times
· Salary will vary given previous experience and education
