Position Summary
The RN Case Manager is responsible for providing nursing oversight and case management services to individuals enrolled in the Texas Home and Community-based Services (HCS) Program. The RN ensures quality medical care, regulatory compliance, delegation of nursing tasks, staff education, and coordination with physicians, pharmacies, therapists, hospitals, and interdisciplinary teams.
Essential Responsibilities

Complete initial and ongoing nursing assessments.

Develop, implement, and monitor individualized nursing care plans.

Conduct RN Delegation in accordance with Texas Board of Nursing rules.

Provide client-specific training to Direct Support Professionals (DSPs).

Monitor the health and safety of assigned individuals.

Review physician orders and ensure implementation.

Coordinate medical appointments, hospital discharges, and follow-up care.

Communicate with families, guardians, service coordinators, pharmacies, DSP, group home managers, co-workers and HHSC

Participate in Interdisciplinary Team (IDT) meetings.

Monitor medication administration and documentation.

Review nursing documentation for accuracy and compliance.

Assist with incident reviews and health-related investigations.

Provide guidance and education to supervisors and direct care staff.

Maintain accurate electronic and paper medical records.

Assist during state surveys, audits, and quality assurance reviews.

Participate in the agency's on-call rotation as needed.
Qualifications
Required
-
Current Texas Registered Nurse (RN) license in good standing.

Valid Texas driver's license and reliable transportation.

Current CPR certification.
-
Excellent communication and organizational skills.

Strong computer skills including Microsoft Office.

Ability to work independently while collaborating with a multidisciplinary team.
Preferred
-
Experience working with individuals with Intellectual and Developmental Disabilities (IDD).

Experience in the HCS or related waiver programs.
-
Knowledge of Texas HHS and HHSC regulations.

Experience with RN Delegation and BON requirements.

Case management experience.

Active Registered Nurse (RM) License in Texas, in good standing

Minimum of 1-2 years of nursing experience (IDD, long term care, or community health preferred)

Valid driver's license and reliable transportation

Ability to travel within assigned caseload area (Dallas, Blach Springs, Lancaster, etc.)
