Overview
The Case Manager assesses client’s needs to determine community services to be provided and helps them access the benefits provided by Sunnyvale Community Services (SCS) and partner agencies in Santa Clara County. The program includes conducting comprehensive in-home assessments and develops (wellness) care plans for these clients. The Homebound Case Manager is responsible for supporting older adults and individuals with disabilities who are considered homebound by providing case management, connecting clients to community resources, and promoting long-term housing and financial stability.
Responsibilities
- Conducts case management services for individuals age 55+ who have been referred by the Homebound Case Management Program and assists them in accessing resources in the community.
- Responds to referrals from caseworkers and referral agencies within 24 hours and conducts an individual comprehensive assessment in the client’s home and develops an Individual Care Plan for each client seen in the Homebound Case Management program.
- Maintains case note records and an awareness of participants’ needs and connect them to appropriate services.
- Maintain client follow-up to ensure participant has received adequate services.
- Directly refers clients to other appropriate services within the community by providing advocacy, information and assistance, and act as liaison with other social service agencies that provide services for older adults.
Outreach and Partnership
- Develop and maintain collaborative relationships with community organizations, healthcare providers, schools, and government agencies.
- Conduct outreach and presentations to increase awareness of SCS services and available services.
- Stay informed of changes to SCS programs and community resources.
Program Administration
- Maintains program statistics including entering of client demographics, care plan, information and assistance data and services provided, and outcomes.
- Prepares and completes required reports within required period.
- Meets the goals and objectives to ensure contract compliance
Knowledge, Skills & Abilities
- Knowledge and comfort working with a diverse population including but limited to the unhoused, developmentally disabled, substance abuse, and cognitive disability.
- Ability to build trusting relationships with individuals from diverse cultural and socioeconomic backgrounds.
- Bilingual English/Spanish/Mandarin preferred
- Strong verbal and written communication skills
- Proficiency with Microsoft Office and electronic case management systems (Salesforce & HMIS preferred)
- Driver’s License and vehicle required to conduct home visits (Mileage reimbursement will be provided)
- Ability to work independently, manage multiple priorities, maintain confidentiality, and lift-up to 25 pounds
- Other duties assigned by supervisor to help support the program and agency needs
Education & Experience
- Bachelor's degree in social work, Human Services, Psychology, Public Administration, or a related field preferred.
- One to three years of experience in case management, social services, or a related field, or an equivalent combination of education and experience.
Pay: $27.00 - $30.00 per hour
