Job Summary
We are seeking a dedicated and compassionate Hospice LPN Visit Nurse to provide high-quality nursing care to patients in their homes. The ideal candidate will possess a strong background in various clinical settings, including hospice, critical care, or long-term care, and demonstrate excellent patient assessment and communication skills. As a vital member of our hospice team, you will deliver personalized care that respects patient dignity and promotes comfort during their end-of-life journey.
Qualification:
· LPN with Current Minnesota License
· Minimum 2 years experience in Geriatrics, Home Care or Hospice
Accountable to:
Hospice Clinical Manager
Knowledge and Skills Required:
· Knowledge of nursing practices and principles
· Ability to establish and maintain constructive relationship with clients and professionals
· Ability to maintain interest in and project sympathetic and cheerful attitude toward the elderly and disabled population
· Ability to exercise tact and good judgement
· Ability to facilitate positive morale
· Ability to collect and organize data
· Computer knowledgeable with keyboarding skills necessary to perform clinical charting on a computer

· Remain up-to-date regarding agency policies and procedures and perform all functions in accordance with Triniti Hospice policies and procedures
· Provide the highest quality of customer service consistent with Triniti values and standards.
· Maintain always client confidentiality as per the Health Insurance Portability and Accountability Act (HIPAA).
1. Attendance and reliability for undisrupted care and services to clients
2. May provide skilled nursing care to either Home Care or Hospice patients
3. Provide skilled nursing care interventions, according to the individual care plan as developed by agency RN or ordered by a licensed provider.
4. Promotes, maintains and restores health using appropriate, preventative, rehabilitative and /or palliative measures by following the clients care plan.
5. Report changes in health status, responses to interventions, and any new orders received to RN Case Manager timely
6. Monitors client’s vital signs, and general health status / condition.
7. Obtains and implements physician orders
8. Reorders meds as needed
9. Communicates to RN any changes in client condition and/or orders
10. Collaborates with family and other professionals to best coordinate care
11. Maintains and completes accurate documentation in a timely manner:
a. Point of Care documentation will be completed with the expectation that at least 80% of the documentation is done in the home
b. Subsequent visits should be completed and synced in on the same day.
12. Schedules/completes visits during regular business hours and per weekend/holiday rotation schedule
14. Participates in case conferences and/or IDT meetings if applicable
15. Attends staff meetings and educational programs as required
16. Follows State and Federal Regulations for Home Care and Hospice
17. Other duties as assigned
Reliable Transpiration Necessary
Work Environment:
· Requires daily travel by automobile in all weather conditions
· Occasional exposure to unclean or unsanitary conditions during course of home visits including cats, dogs, smokers.
· Smoke free office
Physical Demands and Working Conditions:
Ability to lift fifty (50) pounds occasionally. Moving, lifting or transferring of clients may involve lifting to 50 pounds less than occasionally. Ability to bend, stoop, stretch, squat, twist, sit and reach. Fine motor skills and viewing of digital monitor.

Pay: $31.00 - $34.00 per hour
