Overview
We are seeking a highly skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our healthcare team at United Community Health Center. The ideal candidate will possess extensive experience in behavioral health, with a strong background in patient assessment, psychopharmacology, and mental health treatment across diverse populations. This role offers an opportunity to provide comprehensive psychiatric care within a multidisciplinary setting, utilizing advanced clinical skills and a broad knowledge of medical systems and procedures. The PMHNP will collaborate with physicians, therapists, and other healthcare professionals to deliver patient-centered mental health services, including medication management, therapy, and crisis intervention.
Role: Work within an integrated behavioral health and primary care clinic relationship to provide specialized and advanced behavioral health care to clients of all ages. Addresses the mental health needs of behavioral health clients including psychiatric evaluation of clients, prescribing and managing psychotropic medications, as well as care management within an integrated model with close coordination with other behavioral health and primary medical providers; assessing and assisting with staff training needs.
Essential Duties and Responsibilities:
- Oversees the study and treatment of patients with emotional, mental, or psychological disorders.
- Formulates mental health treatment plan and manages acute and chronic mental health needs within an integrated model of care.
- Conducts mental health histories and assessments.
- Manages and directs the activities of behavioral health services.
- Ensures all behavioral health operations are in compliance with clinical guidelines or has evidence to support efficacy. May be responsible for outpatient treatment.
- Manages the Behavioral Health department within an integrated FQHC facility to help the Executive Director meet organizational goals.
- Creates functional strategies and specific objectives for the department and develops budgets/policies/procedures to support the functional infrastructure.
- Deep knowledge of the behavioral health field and solid knowledge of the overall behavioral health departmental function.
- Work with direct medical care peers to collaborate on the patients overall well-being, both physical and mental.
- Provides client education to promote wellness and support disease management process.
- Prescribes and dispenses medications.
- Document accurately all assessments, interventions, client/family responses, medication dispensed or prescribed, procedures, treatments and other data in medical record via process adopted by clinic.
- Completes timely and accurate charge ticket to support efficient and appropriate collection of fees from patients and insurance companies.
- Orders Laboratory and other special diagnostic test and evaluates data obtained.
- Communicates test results and follow-up to client.
- Administers psychotropic medications and therapeutic agents, necessary to implement treatment, disease prevention, or rehabilitation regimen ordered and within the scope of licensure.
- Provides or participates in education of the client and family by assessing learning needs, formulating a teaching plan and initiating a teaching plan to meet the needs of the client.
- Coordinates client care by triaging phone calls regarding urgency of appointments, and schedules hospital admissions and testing.
- Maintains knowledge of available community resources.
- Uses all available resources for diagnosis and treatment in an appropriate and cost efficient manner.
- Maintains a clean, safe and efficient unit environment for clients, visitors and staff.
- Additional responsibilities may include hospital rounds or on call / after hours coverage to ensure patient health and safety is maintained by verifying continued care is available and in place.
- Completes all other assigned projects and duties as assigned.

Experience: One to three years of similar or related experience preferred.
Education: Graduate of accredited PMHNP Program.
