Designated Coordinator / Designated Manager (DC/DM) job summary
The Designated Coordinator / Designated Manager (DC/DM) is responsible for providing compliance oversight, program coordination, quality assurance, documentation management, and person-centered planning support for Home and Community-Based Services (HCBS) providers operating under Minnesota Statutes Chapters 245D.
The DC/DM serves as a compliance resource to providers, staff, and stakeholders by ensuring services are delivered in accordance with Minnesota licensing requirements, person-centered practices, and organizational policies and procedures.
This position requires strong leadership, communication, documentation, and organizational skills, as well as the ability to work independently while managing multiple provider accounts and compliance projects.
Primary ResponsibilitiesCompliance & Regulatory Oversight
- Ensure compliance with Minnesota Statutes Chapters 245A and 245D.
- Conduct internal compliance reviews and audits.
- Assist providers with licensing readiness and regulatory compliance.
- Monitor corrective action plans and compliance improvement activities.
- Review incident and emergency reports and assist with follow-up actions.
- Support providers during licensing reviews, investigations, and audits.
Person-Centered Planning & Service Coordination
- Conduct client intakes, admissions, and assessments.
- Develop and review Support Plans and Support Plan Addendums.
- Complete and review Self-Management Assessments (SMA).
- Complete and review Individual Abuse Prevention Plans (IAPP).
- Coordinate services with case managers, guardians, legal representatives, and interdisciplinary teams.
- Monitor implementation of person-centered services and outcomes.
Documentation & File Management
- Review client files for compliance and completeness.
- Complete annual reviews, Forty-Five (45) Day Reviews, and Sixty (60) Day Reviews.
- Review progress notes and service documentation.
- Ensure all required signatures and documentation are maintained.
- Prepare reports and documentation required by regulatory agencies.
- Assist providers with documentation corrections and audit preparation.
Staff Training & Competency
- Conduct employee orientation and annual training reviews.
- Conduct and document First Supervised Direct Contact with Persons Served.
- Complete Staff Orientation Forms and Staff Annual Training Forms.
- Track employee training and compliance deadlines.
- Train staff on documentation standards, progress note writing, and Minnesota 245D requirements.
- Monitor staff competency and provide coaching and corrective guidance when necessary.
Quality Assurance & Program Support
- Conduct quality assurance reviews and compliance monitoring activities.
- Identify compliance concerns and recommend corrective actions.
- Assist providers with policy and procedure implementation.
- Support continuous quality improvement initiatives.
- Ensure providers maintain compliance with Minnesota licensing requirements and industry best practices.
QualificationsRequired
- Meet Designated Coordinator and/or Designated Manager qualifications under Minnesota Statutes §245D.081.
- Minimum of two (2) years of experience in Minnesota 245D services.
- Strong knowledge of Minnesota 245D licensing requirements.
- Excellent written and verbal communication skills.
- Strong organizational and time-management skills.
- Ability to work independently and manage multiple responsibilities.
- Valid driver's license and reliable transportation.
- Ability to pass DHS Background Study requirements.
- Authorization to work in the United States.
- Fluent in written and spoken English.
Preferred
- Bachelor's Degree in Human Services, Social Work, Psychology, Healthcare Administration, Special Education, or a related field.
- Supervisory experience in a 245D licensed setting.
- Experience conducting compliance audits and quality assurance reviews.
- Experience completing Support Plans, Support Plan Addendums, SMAs, IAPPs, annual reviews, 45-Day Reviews, and 60-Day Reviews.
Work Schedule
- Hybrid Schedule: Three (2) days remote and two (3) days in-office.
- Occasional travel throughout Minnesota for provider visits, client meetings, audits, and compliance reviews.
Compensation
- $28.00 per hour
