Bilingual Care Coordinator (English/Spanish)
Location: Wyomissing PA.
Employment Type: Full-Time
Schedule: Monday through Friday, 8:00 AM – 5:00 PM
Position Summary
We are seeking a highly motivated and experienced Bilingual Care Coordinator (English/Spanish) to join our Home Health team. The ideal candidate will have a strong background in home healthcare coordination, excellent communication skills, and experience working with caregivers, patients, and clinical staff to ensure high-quality care and service delivery.
Responsibilities
- Coordinate and manage patient care services in accordance with physician orders and agency policies.
- Serve as a liaison between patients, families, caregivers, clinicians, and referral sources.
- Schedule and coordinate Home Health Aides (HHAs) and other care staff.
- Maintain accurate patient records and documentation.
- Monitor patient service plans and ensure continuity of care.
- Assist with intake, staffing, and ongoing patient care coordination.
- Communicate effectively in both English and Spanish with patients, families, and staff.
- Utilize agency software systems to manage scheduling, documentation, and reporting.
- Ensure compliance with all applicable regulations and company policies.
QualificationsRequired:
- High School Diploma or GED.
- Fluent in English and Spanish (written and verbal).
- Minimum of 2 years of experience in a Home Health agency.
- Experience coordinating and scheduling Home Health Aides (HHAs).
- Experience using the WellSky Home Health software system.
- Strong organizational, multitasking, and communication skills.
- Proficiency with Microsoft Office and general computer applications.
Preferred:
- Some college coursework or an Associate's degree in Healthcare Administration, Business Administration, Nursing, or a related field.
- Additional experience in patient care coordination, healthcare administration, or case management.
