Albany Hospice is looking for an Intake Coordinator to be the welcoming first point of contact for patients, families, referral partners, and healthcare providers. ✨ Manage daily patient referrals and admissions
✨ Verify physician orders, insurance, and authorizations
✨ Maintain accurate patient records and intake documentation
✨ Build strong relationships with referral sources and community partners
✨ Track referral activity and support service growth opportunities
✨ Ensure HIPAA compliance and regulatory standards are met
✨ Represent Albany Hospice with professionalism and compassion
✔ High School Diploma or GED
✔ 2+ years of healthcare administrative experience preferred
✔ Knowledge of medical terminology and insurance processes
✔ Strong communication, organization, and computer skills
✔ Ability to multitask in a fast-paced environment
✔ Valid driver's license and current insurance
