Job Title -LVN / LPN – Step-Down / Progressive Care
Location: Dayton, OH (More than 50 miles)
Start Date: September 8, 2026
Contract Length: 20 weeks with possible extension (BI02)
Shift: Nights, 7:00 PM – 7:30 AM
Schedule: 3 × 12-hour shifts 0R 4*12
Hours: 36 or 48 hours per week available
Weekend Requirement: Every other weekend and holidays as scheduled
Job Summary
We are seeking an experienced LVN/LPN for a high-acuity Step-Down / Progressive Care unit in Dayton, Ohio. The ideal candidate will have strong experience caring for medically complex adult and geriatric patients, including patients requiring ventilator support, telemetry monitoring, BiPAP, tracheostomy care, and cardiac/insulin/heparin drips.
This is a fast-paced environment that emphasizes teamwork, safe patient care, and collaboration across nursing and ancillary services.
Key Responsibilities
- Provide nursing care to adult and geriatric patients in a Step-Down/Progressive Care setting.
- Monitor vital signs, telemetry, oxygenation, and changes in patient condition.
- Care for patients requiring ventilators, tracheostomies, BiPAP/CPAP, and high oxygen requirements.
- Monitor and manage patients receiving cardiac, heparin, insulin, and other appropriate IV drips.
- Perform medication administration and IV therapy.
- Monitor blood glucose and manage patients with DKA and other metabolic conditions.
- Perform wound, drain, chest tube, PleurX, and catheter care.
- Maintain accurate intake/output records.
- Follow isolation and PPE precautions.
- Communicate changes in patient condition to the RN/physician.
- Participate in emergency response and CPR when required.
- Collaborate with RNs, PCTs, respiratory therapists, physicians, and other healthcare professionals.
- Participate in team nursing or primary nursing based on unit needs.
Required Qualifications
- Active LVN/LPN license
- Minimum 1 year of nursing experience; 1.5+ years preferred
- Step-Down/PCU experience REQUIRED
- Ventilator experience REQUIRED
- Epic EMR experience REQUIRED
- BLS certification REQUIRED
- ACLS certification REQUIRED
- Ability to read and interpret telemetry/EKG rhythms
- Documented experience with approximately 1:5 patient ratios
- Travel nursing experience preferred
Important Experience Requirement
ICU experience alone does not qualify as Step-Down/PCU experience. Candidates must have actual Step-Down or PCU experience, including documented experience managing a 1:5 patient ratio.

Patient Population
The unit primarily cares for patients with:
- COPD
- CHF
- Pneumonia
- Sepsis
- Post-COVID conditions
- Diabetic Ketoacidosis (DKA)
- Complex respiratory conditions
- Tracheostomy and ventilator needs
- BiPAP/CPAP requirements
- High oxygen requirements
Patient Ratio
- Target ratio: 1:4
- Ratios may increase to 1:5–6 depending on patient acuity and staffing.
