Position:- Care Management Specialist-Waiver
Location:- Oak Forest, IL (Applicant accepted from South, North and West sides, IL)
Rate:- $40-$43/hr.
Working hour :-40 hours per week
(Travel in South, North and West sides, and have to take assessments at member's at home) - 60-70% TRAVEL & 30-40% REMOTE.

- License required LPC, LCSW, LCPCs and RNs
- 1+ year Quality improvement experience.
- 3+ year Clinical experience
Job Overview:
We are seeking a skilled and compassionate professional to support medical management and health education initiatives for members enrolled in government healthcare programs. This role focuses on improving patient outcomes through clinical guidance, data-driven insights, and collaboration with internal and external stakeholders.
Key Responsibilities:
- Conduct medical management and health education programs for members
- Analyze and interpret clinical and regulatory data to support reporting requirements
- Serve as a primary point of contact for members, addressing medical needs and care coordination
- Collaborate with physicians, care teams, and external vendors to ensure effective service delivery
- Provide clinical insights and recommendations to improve care quality and outcomes
- Support wellness initiatives and managed care programs
Required Qualifications:
- Active, unrestricted license in one of the following:
- Registered Nurse (RN) with 3+ years of direct clinical experience
- Licensed Professional Counselor (LPC)
- Licensed Master Social Worker (LMSW) with required clinical experience
- Minimum 3 years of experience in wellness or managed care, including interaction with members and physicians
- Strong understanding of healthcare systems, wellness programs, and industry trends
- Excellent verbal and written communication skills
- Experience in medical data analysis and reporting
- Proficiency in MS Office (Word, Excel, PowerPoint) and web-based applications
- Valid driver’s license, reliable transportation, and willingness to travel within the assigned territory
Preferred Qualifications:
- Clinical experience in patient education or condition management
- Transition of Care experience
- Experience managing complex or catastrophic cases
- Bilingual (English/Spanish)
- Certification in Case Management, Training, Project Management, or related healthcare fields
