Duties
- Assist clients with Activities of Daily Living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods
- Monitor patients’ vital signs and observe their condition to ensure safety and health, documenting changes accurately in care plans
- Administer medication following HIPAA regulations and client-specific protocols, ensuring proper documentation and adherence to safety standards
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote healthy eating habits
- Support clients during community outings or transportation using personal transportation assistance, ensuring safety at all times
- Implement individualized care plans designed by healthcare professionals, including dementia care, Alzheimer’s care, memory care, autism support, and hospice care
- Maintain a clean and safe environment by performing routine cleaning tasks and adhering to infection control protocols
Experience
- Prior experience working with individuals with disabilities or developmental challenges such as autism or intellectual disabilities support is highly preferred
- Group home experience or assisted living experience demonstrating familiarity with residential care settings
- Knowledge of patient observation techniques, patient monitoring, and vital signs measurement in home health or nursing home environments
- Hands-on experience with patient transfer methods like Hoyer lifts or other manual handling equipment
- Certification in CPR and First Aid is required; additional training in dementia or Alzheimer’s care is a plus
- Experience working within HIPAA guidelines to protect client confidentiality and privacy
- Background in geriatrics, home & community care, long-term care, or hospice enhances your ability to provide comprehensive support
