Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and record vital signs, medication administration, and patient observations to ensure health and safety
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Support clients with mobility using equipment like Hoyer lifts or assistive devices, ensuring safe transfers and positioning
- Maintain cleanliness of living spaces through light housekeeping tasks including cleaning, laundry, and organizing
- Follow care plans meticulously, documenting progress and communicating updates to healthcare teams or family members
- Provide companionship and emotional support, engaging clients in social activities to promote mental well-being
Skills
- Proven experience in home & community care or assisted living environments with a strong understanding of patient care principles
- Knowledge of HIPAA regulations to ensure confidentiality and privacy of client information
- Certifications such as CPR, First Aid, and training in dementia or Alzheimer's care are highly valued
- Ability to observe and report changes in patient condition accurately and promptly
- Experience working with individuals with developmental disabilities, autism, or memory-related conditions like dementia or Alzheimer's disease
- Physical stamina for heavy lifting, assisting with transfers, and long periods of standing or walking
- Strong communication skills for collaborating with healthcare professionals, families, and clients effectively
