Job Title/Position: Certified Hospice Aide
Reports To: Case Manager and Director of Patient Care Services
AMAYA Hospice is seeking compassionate Hospice Aides to assist hospice patients in the DFW metroplex and surrounding counties with ADLs. The Certified Hospice Aide (CHA) provides compassionate, patient-centered care to individuals facing terminal illnesses. Under the supervision of a Registered Nurse, the CHA delivers personal care, emotional support, and assistance with daily living activities to hospice patients in their homes, care facilities, or other designated locations. The role requires adherence to the hospice philosophy and a commitment to ensuring patients' comfort, dignity, and quality of life.
Key Responsibilities:
1. Patient Care
a. Provide personal care, such as bathing, dressing, grooming, oral hygiene, toileting and skin care.
b. Assist with ambulation, transfers and mobility needs, including use of wheelchair or other assistive devices.
c. Monitor and report changes in patients’ physical or emotional conditions to RN or supervisor
d. Provide companionship and emotional support to patients and their families
e. Respect and uphold patients’ dignity, cultural values and preferences.
2. Environment Maintenance
a. Maintain a clean and safe living environment by assisting with light housekeeping tasks, such as changing bed linens and laundry.
b. Ensure medical equipment is clean and functional, reporting any issues promptly.
3. Care Coordination
a. Coordinate with other team members, such as nurse, social workers, and chaplains to meet patient’s needs holistically.
b. Collaborate with the interdisciplinary teams to ensure continuity of care and adherence to the patient’s care plan.
4. Documentation
a. Accurately document care provided and any observations related to patient’s condition in accordance with agency policies and procedures
Qualifications
1. Is at least 18 years of age
2. Current and valid Driver’s License
3. Current and valid CPR certification
4. Current and valid CNA license and who has successfully completed a state established or other training program that meets the requirements of S 484.36(a) and a competency evaluation program, or state licensure program that meets the requirements of S 484.36(b), or a competency evaluation program or state licensure program that meets the requirements of S 484.36(b).
5. Licensed driver with automobile that is unsured in accordance with organization requirements and is in good working condition
6. Previous experience in hospice preferred.
7. Satisfactory references from previous employers
8. Ability to pass a background check
9. Is self-directing with the ability to work with little direct supervision. Secure with issues of death/dying. Provides a calm manner within a patient’s home

Job Limitations
The hospice aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician’s orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, such as sterile techniques.
