POSITION SUMMARY
The HIV/PrEP Navigator position will ensure patient and community engagement through outreach and care coordination of patients living with or at risk for HIV. This position will improve community awareness, patient care outcomes, and quality of life for this focused population. The HIV/PrEP Navigator provides confidential education as well as facilitates testing and linkage to care to current Adelante patients and community members through external partnerships and events. The HIV/ PrEP Navigator coordinates with clinical team members to ensure that effective patient care services are provided. In this role the HIV/PrEP navigator navigates patients through the patient care delivery system, addresses social drivers of health, coordinates patient care services, and advocates for whole person health care. HIV/Prep Navigator is also able to carry out standing orders and submit prior authorizations as needed on behalf of the prescribing clinicians to expedite care. This role also provides education and support to staff to achieve program and organizational goals.
ESSENTIAL SKILLS AND EXPERIENCE:
- High school diploma or GED from an accredited institution
- Graduation from an approved medical assistant training program
- A minimum of three (3) years’ experience working as a Medical Assistant in a health care setting
- Two (2) years of working experience in a community outreach setting or in a clinical setting, preferred
- Demonstrates cultural competency and ability to relate to people living with HIV/AIDS, gay/bisexual men, trans individuals, as well as individuals from diverse socio-cultural and socioeconomic backgrounds.
- English/Spanish Medical Interpreter Certification required
- Knowledge and experience with the Hispanic/Latino, Black/African American communities as well as the LGBT community.
- Ability to speak with broad cross sections of the community about personal sexual behavior and drug use in a non-judgmental and caring way.
- One (1) year of work and/or volunteer experience providing direct service to individuals from the LGBT community or individuals who are at high risk of contracting HIV or STIs is highly desired.
- Experience with or willingness to learn group facilitation, teaching and/or public presentations is highly desirable.
- Ability to explain rapid HIV tests in compliance with Manufacturer’s instructions (experience preferred).
- Basic understanding of how HIV is transmitted and prevented.
- Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross (cognitive and skills evaluations).
- Maintain a flexible schedule, including evenings and weekends, to support outreach activities.
- Must have reliable transportation for outreach, education, training events, and community presentations.
- Valid Arizona Drivers License and active motor vehicle insurance
- Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations
POSITION REPONSIBILITIES
- Coordinate and maintain the outreach calendar and marketing material inventory.
- Promote HIV education, testing, prevention, and related services through community engagement and patient outreach.
- Support comprehensive patient care and case management within established timelines.
- Maintain patient confidentiality and ensure compliance with HIPAA and organizational policies.
- Accept program referrals and provide timely care coordination and navigation services for eligible patients.
- Coordinate referrals to internal and external healthcare, social, and support services.
- Facilitate patient intake and access to appropriate medical and supportive services.
- Address social determinants of health, including transportation, housing, food access, and other barriers to care.
- Monitor patient engagement, follow-up activities, and health outcomes.
- Conduct inbound and outbound communication for care coordination and patient support.
- Assist patients with insurance navigation, medication access, prior authorizations, and financial assistance programs.
- Accurately document all patient interactions and care coordination activities.
- Maintain ongoing patient contact to support treatment goals and identify emerging needs.
- Communicate patient concerns, clinical changes, or urgent needs to the care team.
- Advocate for patients and serve as a liaison between patients, providers, and community resources.
- Provide patient education within the Community Health Worker scope of practice.
- Build collaborative relationships with patients, families, staff, and community partners.
- Support individuals living with HIV in overcoming barriers to care, maintaining ART adherence, and achieving viral suppression with antiretroviral therapy (ART) adherence and achieve viral suppression.
Pay: $20.00 - $30.00 per hour
