LVN Field Nurse (Los Angeles and South Bay area)
** Hospice Experience and Bilingual in Spanish Preferred **
Work Schedule: Monday-Friday 8:30am-5:00pm.

Agape Hospice & Palliative Care is seeking a LVN Field Nurse to support patients throughout Los Angeles and South Bay. Based out of our Torrance office (with excellent parking), we are a clinician-owned, Joint Commission–accredited hospice known for exceptional patient care, teamwork, and a supportive work culture.
This role is ideal for an LVN who is responsive, flexible, and comfortable traveling between locations to meet urgent patient needs. You’ll play a critical role in ensuring comfort, safety, and continuity of care during evenings, nights, weekends, and urgent situations. About the Role
As an LVN Field Nurse, you will travel throughout Los Angeles and the South Bay providing scheduled hospice nursing visits during regular weekday business hours. Working under the supervision of an RN, you will deliver skilled, compassionate hospice care in patient homes, assisted living communities, and skilled nursing facilities.
Your presence brings consistency, clinical expertise, and comfort to patients and families during their hospice journey.
Your Responsibilities Include:
- Traveling throughout Los Angeles and the South Bay to conduct scheduled patient visits
- Performing skilled LVN nursing visits under the direction and supervision of an RN
- Providing routine and PRN visits as assigned during regular business hours
- Assisting with symptom management, medication administration/support, and comfort-focused care
- Monitoring patient condition and promptly reporting changes to the RN and interdisciplinary team
- Supporting care plan implementation and ensuring continuity of care
- Providing compassionate education and emotional support to patients and families
- Accurately documenting visits and care in our EMR system (HospiceMD)
- Maintaining hospice standards, infection control practices, and patient safety at all times
- Participating in interdisciplinary team collaboration and care coordination
This is meaningful, hands-on hospice work where you build ongoing relationships with patients and families while supporting them with dignity, stability, and compassionate clinical care during regular weekday hours.
Qualifications
- Current, active LVN license in the state of California
- Minimum of 3 year of LVN experience (hospice experience strongly preferred)
- Comfortable working independently in a travel-based field role
- Strong clinical judgment, communication, and documentation skills
- Reliable transportation, valid auto insurance, and clean driving record
- Ability to work evenings, nights, weekends, and holidays as scheduled
- Familiarity with Los Angeles and South Bay preferred
- Bilingual in Spanish highly preferred
