Lynda's Home Health Care is a growing home health agency dedicated to providing compassionate, high-quality care to patients in the comfort of their homes. We are seeking a dedicated and experienced Speech-Language Pathologist (SLP) to join our team and help improve the communication, cognitive, and swallowing abilities of our patients.
Position Summary
The Speech-Language Pathologist is responsible for evaluating, diagnosing, and treating speech, language, cognitive, communication, and swallowing disorders in patients receiving home health services. The SLP will work collaboratively with physicians, nurses, physical therapists, occupational therapists, social workers, patients, and family members to develop and implement individualized treatment plans.
Responsibilities
- Perform comprehensive speech, language, cognitive, and swallowing evaluations.
- Develop individualized treatment plans based on patient needs and physician orders.
- Provide skilled speech therapy services in patients' homes.
- Assess and treat patients with dysphagia, aphasia, cognitive deficits, speech disorders, and communication impairments.
- Educate patients, family members, and caregivers regarding treatment plans and home exercise programs.
- Monitor patient progress and modify treatment goals as appropriate.
- Complete all documentation accurately and timely in accordance with agency policies and Medicare guidelines.
- Communicate effectively with physicians and interdisciplinary team members regarding patient status and progress.
- Participate in care coordination and case conferences as required.
- Maintain compliance with federal, state, and agency regulations.
Qualifications
- Master's Degree in Speech-Language Pathology from an accredited program.
- Active California Speech-Language Pathologist License.
- Certificate of Clinical Competence (CCC-SLP) preferred.
- Minimum one (1) year of experience as a Speech-Language Pathologist preferred.
- Home health experience preferred but not required.
- Current CPR/BLS Certification.
- Valid California Driver's License.
- Reliable transportation and current auto insurance.
- Strong clinical assessment and documentation skills.
- Excellent communication and interpersonal skills.

Service Area
This position primarily serves patients in Santa Monica and surrounding areas of Los Angeles County. We also have patients in Orange County if you are willing to make the trip.
