We are seeking a motivated, relationship-driven Patient Care Coordinator to guide prospective and current patients through their wellness journey. This role is ideal for someone who excels at building trust, communicating value, and helping patients confidently move forward with recommended care plans.
As a key member of our team, you will serve as the bridge between our providers and patients, ensuring individuals understand their treatment recommendations, financial options, and next steps while providing an exceptional patient experience.

Present provider-recommended treatment plans and services to patients.

Confidently discuss packages and benefits to patients

Follow up with prospective patients to answer questions

Track patient interactions and plan metrics within practice systems.

Build strong relationships that encourage patient retention.

Guide patients through onboarding and scheduling processes

Provide exceptional customer service via phone, text, and email interactions

Ensure patients feel informed, supported, and confident in their care decisions.

Coordinate appointments, testing, and treatment timelines.

Address concerns promptly and professionally while maintaining a positive patient experience.

Maintain accurate patient records and documentation.

Process payments and assist patients with financing applications when applicable.

Collaborate closely with providers and clinical staff regarding patient recommendations and care plans.

Support front-office operations as needed.

2+ years of experience in sales, patient coordination, customer success, healthcare, wellness, or a related field.

Strong communication, relationship-building, and objection-handling skills.

Comfortable discussing pricing and financial commitments with patients.

Highly organized with strong attention to detail.

Experience with CRM systems, EMRs, or patient management software preferred.

Passion for holistic, functional, and preventive healthcare.

Compensation: Base pay plus bonuses as applicable
