Practice Wellness LLC is a bilingual trusted community mental health located in Orlando, FL. We are seeking a Licensed Social Worker (LSW) to provide high-quality mental health services to individuals and families to join our professional team. We offer flexible full-time and part-time positions, including opportunities for remote and hybrid work. Our mission is to support healing, growth, and emotional wellness in the communities we serve.
Key Responsibilities:
- Conduct comprehensive mental health assessments and biopsychosocial evaluations
- Develop and implement individualized treatment plans
- Make appropriate referrals to internal and external services such as psychiatry, Psychosocial Rehabilitation (PSR), Targeted Case Management (TCM), or community support programs.
- Participate in discharge planning and facilitate smooth care transitions.
- Supervise registered interns and/or unlicensed mental health counselors under Florida supervision standards.
- Collect and document client information through interviews, observations, and clinical tools.
- Maintain strict confidentiality and compliance with HIPAA regarding client records.
- Collaborate with interdisciplinary professionals through team meetings and clinical supervision to coordinate care.
- Ensure accurate and timely completion of all clinical documentation and progress notes.
Qualifications:
- Active Florida license as LSW (required).
- Medicaid Provider Number (preferred).
- Minimum 1 year of clinical experience in individual, family, or couples counseling across diverse populations (adolescents, adults, older adults).
- Bilingual in English, Creon and/or Spanish. (A plus)
- Strong interpersonal, organizational, and documentation skills.
- Supervisory experience or willingness to supervise interns (preferred but not required).
Pay: $60.00 - $70.00 per hour
