Travel Home Health Registered Nurse (RN) – Rutland, VT | 13-Week Contract
MTK Healthcare is seeking an experienced Home Health Registered Nurse (RN) for a 13-week travel assignment with VNA Hospice of the Southwest Region in Rutland, Vermont.
Position Details

- Start Date: July 27, 2026
- Contract Length: 13 Weeks
- Schedule: Monday–Friday, 8:00 AM – 4:00 PM
- Guaranteed Hours: 37 hours/week
- On-Call: Some on-call and weekend coverage required
- Holiday Coverage: Required
- EMR: McKesson
Responsibilities
- Perform comprehensive patient assessments in the home setting.
- Develop, implement, and manage individualized plans of care.
- Complete accurate and timely OASIS documentation.
- Provide skilled nursing care, medication management, wound care, IV therapy, and patient education.
- Coordinate care with physicians and the interdisciplinary healthcare team.
- Monitor patient progress and revise care plans as needed.
- Maintain compliance with agency, state, and federal regulations.
- Educate patients and caregivers on disease management, medications, and home care.
Qualifications
- Active Registered Nurse (RN) license (Vermont license required upon acceptance).
- Minimum of 2 years of RN experience.
- Home Health and OASIS experience required.
- Experience with wound care, IV therapy, wound VACs, and case management.
- Strong communication, documentation, and organizational skills.
- Ability to work independently in a home health environment.
- Valid driver's license and reliable transportation.
