Nayar Hospice is seeking a compassionate and dedicated PRN Hospice Social Worker (MSW) to support patients and families during one of the most important transitions of their lives.

Position Overview
As a Hospice Social Worker, you will provide emotional, psychosocial, and bereavement support to patients and families. This role is about more than documentation—it’s about presence, compassion, and guiding families with dignity and respect.
Responsibilities
- Provide emotional and psychosocial support to patients and families
- Assist with end-of-life planning and decision-making
- Offer bereavement support before and after loss
- Collaborate with the interdisciplinary team
- Complete assessments, care plans, and documentation
- Coordinate with volunteers and community resources
Qualifications
- Master’s Degree in Social Work (MSW) required
- Hospice or healthcare experience preferred
- Strong communication and documentation skills
- Ability to handle sensitive conversations with compassion
