Independent Living Case Manager - HHYS

The Independent Living Case Manager provides case management services for young adults who have recommitted to the state. Case management services are goal oriented and time limited. The goals are to ensure the safety of the child and community while developing a structured treatment plan for each individual child in a timely manner.
Essential Job Responsibilities:
- Provide case management services to Independent Youth ages 18-21 that have entered into Program.
- Develop, with the Youth, IL Team, a goal oriented, time limited Treatment Plan and Skills Assessment within thirty days after the youth is referred to the program along with the information the worker gathered at the time of intake and information gathered from other available sources.
- Prepare reports and reviews in a timely, clear, and concise manner per service manual guidelines, including individual treatment plans or person-centered plans, contact logs, quarterly reviews, safety checks, supervision notes, discharge summaries, and other assigned reports, utilizing computer-based clinical management software and other network software
- Complete all documentation within agency deadlines, and state deadlines
- Provide services in a manner which empowers clients and follow professionally recognized standards of ethical practice.
- Participate in the planning and delivery of services regarding admission, treatment, and discharge. This includes coordination and facilitation of Child and Family Team Meetings and attend clinical staffing and meetings
- Maintain weekly face to face contact with youth and monthly contacts with social workers, therapists, school, and other providers.
- Assist youth in learning independent living skills.
- Keep current and accurate client records, case files, record notes and medical records.
- Meet weekly with supervisor to consult on cases.
- Must complete 24 additional training hours annually.
- Be knowledgeable of community resources and treatment modalities.
- Monitor that necessary medical/dental care and appropriate schooling, including transportation arrangements to such services is provided. Collaborate with youth and care providers to monitor all doctor's appointments and medications.
- Demonstrate an understanding of the effects of trauma, including separation and loss, neglect, physical and sexual abuse has on a child. Assist child and develop treatment plan to enhance their well-being.
- Participate in on-call systems as required by program.
- Perform other duties as assigned
Credentials:
A person employed as Independent Living Case Manager shall possess the following qualifications:
- Possession of at least a Bachelor’s degree in human service field; previous experience in case management is preferred.
- Be willing to become certified as a Targeted Case Manager. Training paid for by the agency
Physical Requirements:
- Perform duties such as lifting furniture in and out of client apartments as needed. (greater than 50 pounds with assistance)
- Walk up and down multiple flights of stairs carrying furniture with or without assistance as needed.
