Physical Therapist (PT) – Home Health Case Manager
Location: Chicago – South Side (servicing ZIP codes 60620, 60628, 60643 and surrounding areas)
Schedule: Day shift with rotating weekends and holidays
Sign-On Bonus:
Eligible for up to $10,000 sign-on bonus for external candidates. Must be actively working in the same role at the time of payout.
As a Home Health Physical Therapist, you’ll have the opportunity to manage your own caseload while being supported by a highly collaborative, integrated care team. This role offers autonomy, meaningful patient relationships, and the ability to make a direct impact on patient independence and quality of life in the home setting.

Provide comprehensive physical therapy assessments and develop individualized care plans

Deliver in-home therapy focused on improving mobility, safety, and independence

Serve as a case manager, coordinating care with physicians and interdisciplinary teams

Educate patients and families to support recovery and long-term outcomes

Travel throughout the assigned North Side Chicago territory

Licensed Physical Therapist in Illinois (or eligible)

Minimum 1 year of clinical experience (or strong clinical affiliation experience)

Experience with home-based therapy equipment and assistive devices

Strong communication, documentation, and time management skills

Ability to work independently while collaborating with a broader care team
