Job Overview
Main Street Hospice is searching for Physical, Occupational and Speech Therapists to make visits to the patient home/facility and provide therapy to our terminally ill patients and families. Main Street has need for Therapists in Southern Johnson County, Marion, Shelby, Bartholomew, Brown, Hendricks, Hancock, Monroe and Morgan Counties. Main Street is seeking a PRN/Per Diem PT, OT and ST for this position. This position is ideal for a Therapist to supplement another primary career - it allows our Therapists the flexibility to experience hospice care and supplement their incomes. Main Street Hospice is a very ethical, caring hospice who truly cares about our patients and staff.
Responsibilities
- Conduct therapy assessments to identify the physical and safety needs of the terminally ill patients.
- Develop and implement individualized care plans tailored to the specific needs of the patient and caregiver/family.
- Provide education and support to patients and their families.
- Collaborate with multidisciplinary team to ensure comprehensive care for patients.
- Maintain accurate documentation of client interactions, visit notes, and interdisciplinary meeting prep notes.
Experience
- Must have 1 year of therapy experience in a healthcare setting;
- Must be a Licensed Physical, Occupational or Speech therapist licensed in the State of Indiana.
