Overview
We are seeking a dynamic and clinically experienced Clinical Liaison to serve as the primary point of contact between hospitals, physicians, case managers, and our Skilled Nursing Facility. The Clinical Liaison is responsible for evaluating patient referrals, coordinating admissions, and building strong relationships within the healthcare community to support census growth and continuity of care.
Duties
- Review and assess patient referrals for admission to the SNF, ensuring clinical appropriateness and regulatory compliance
- Collaborate with hospital discharge planners, case managers, and physicians to coordinate smooth patient transitions
- Conduct on-site hospital visits to evaluate patients and promote facility services
- Communicate effectively with internal interdisciplinary teams regarding incoming admissions
- Maintain accurate documentation of referral activity and outcomes
- Develop and maintain relationships with referral sources to drive admissions growth
- Educate patients and families on services provided by the facility
- Participate in marketing and outreach efforts within the healthcare community
- Stay current with Medicare, Medicaid, and insurance guidelines related to admissions
- The ability to gather clinical information and communicate properly with the admissions coordinator.
Qualifications
- Current and active RN or LPN license (preferred)
- Minimum of 2–3 years of clinical experience in a hospital, SNF, or acute care setting
- Previous experience in admissions, case management, or clinical liaison role preferred
- Strong knowledge of post-acute care and insurance criteria
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and manage multiple priorities
- Valid driver’s license and reliable transportation (for hospital visits)
Preferred Skills
- Experience with electronic medical records (EMR) systems
- Marketing or business development experience in healthcare
- Familiarity with local hospital systems and referral networks
Work Environment
- Combination of hospital, office, and facility settings
- Frequent local travel required
