Overview
We are seeking a highly skilled and compassionate Registered Nurse (RN) Case Manager to join our healthcare team.
Job Title: RN Case Manager
Location: Boston, MA
Duration: 13 week contract
Shift: 40 hours per week, day shift
Pay Rate: $2670/week
Responsibilities:
- Collaborates with the multidisciplinary health care team to develop and coordinate the plan of care
- Communicates with patient, family and/or significant health care teams
Qualifications:
- 2 years of experience in case management
- 6+ months experience in discharge planning
- Experience in acute care hospital or teaching organization preferred
- MA RN license in hand
