RN Hospice Case Manager
Full-Time | Monday–Friday | Serving Fort Mill area
Position Summary
Epic Care Health is hiring a compassionate RN Case Manager to join our care team serving patients in the Fort Mill area. This role is ideal for a nurse who values autonomy, meaningful relationships with patients and families, and the support of a collaborative team.
As an RN Case Manager, you’ll be responsible for coordinating and delivering high-quality hospice care to patients in their homes or care facilities. You will play a vital role in managing symptom control, educating families, and ensuring a dignified and comfortable end-of-life experience for each patient.
Position Details:
- Full-time, flexible schedule
- Home-based visits within a designated service area
- Manageable caseload to ensure quality, focused patient care
- Support from LPNs, aides, and administrative staff to streamline your workflow
Responsibilities:
- Provide skilled nursing visits and develop individualized care plans
- Manage pain and symptoms based on each patient’s needs
- Coordinate with interdisciplinary team members including physicians, social workers, chaplains, and CNAs
- Serve as the main point of contact for patients and families
- Maintain accurate clinical documentation in accordance with state, federal, and organizational guidelines

Qualifications:
- Current RN license in the state of South Carolina
- Minimum of one year of hospice or home health experience preferred
- Excellent clinical assessment, communication, and critical thinking skills
- Reliable transportation and valid driver’s license
