Position Overview:
We are seeking a compassionate, organized, and motivated Community-Based Rehabilitation Specialist (CBRS) to join our team. In this role, you will provide support to individuals with mental health challenges in their homes and community settings. Your primary goal will be to help clients build life skills, increase independence, and improve overall quality of life through goal-driven, rehabilitative services.
Key Responsibilities:
- Provide individualized, skill-building interventions in the community to support client treatment goals.
- Assist clients with developing and practicing daily living skills (e.g., budgeting, hygiene, cooking, time management).
- Support clients in navigating housing, employment, education, and community resources.
- Collaborate with therapists, case managers, families, and other team members to ensure holistic care.
- Monitor and document client progress toward goals using established treatment plans.
- Participate in team meetings, trainings, and supervision.
- Maintain professional boundaries, confidentiality, and compliance with agency and Medicaid guidelines.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related human services field (Required)
- Previous experience in mental health, rehabilitation, or social services (Preferred)
- Ability to work independently in community-based settings
- Excellent communication, time management, and problem-solving skills
- Valid driver’s license and reliable transportation
- Ability to pass a background check and clearances as required by state/funding regulations
Job Types: Full-time, Part-time
Pay: $21.00 - $25.00 per hour
