A Brief Overview
Facilitates communication between the patient, their primary care physician and specialists to improve clinical outcomes. Works collaboratively with physician, hospital and community resources to support the patient outcomes. Provides services within and outside of health system campus. Manages and leads assigned clinical teams, and workflows, which can include management and supervision of staff, providing care delivery such as in a clinic environment, or group team lead functions.

Education

Bachelor’s Degree in Nursing (Required)

Master’s Degree in Nursing (Preferred)
Experience
-
3 years experience in disease or care management services with focus on ambulatory care coordination (Required)

General Experience with clinical, hospital-based information systems (Required)
Licenses and Certifications

PA Registered Nurse License (Required) or Multi State Compact RN License (Required)
