General Responsibilities: Provide culturally responsive, trauma-informed services to youth and their families to support safe, stable housing and progress toward self-sufficiency. Work to prevent youth from running away when possible and assist those who are runaway or homeless in accessing emergency shelter and supportive services. Assess whether family reunification is safe and appropriate or help identify alternative safe housing options.
Responsibilities
Provide professional, respectful, and confidential crisis intervention services to youth and their families.
- Assist youth and families in resolving immediate housing crises by connecting them to shelters and other resources. Promote family reunification when safe and appropriate, and engage families in planning meetings, including obtaining parental consent for minors.
- Conduct comprehensive, evidence-based assessments (e.g., biopsychosocial, health/mental health, life skills, trauma, and specialized screenings). Collect required documentation using appropriate releases of information.
- Collaborate with youth and families to develop a Housing Stability Plan focused on safe housing, improved well-being, self-sufficiency, and supportive community connections.
- Maintain accurate and timely documentation, including case notes, service records, and outcome data.
- Work collaboratively as part of a team, supporting colleagues and adhering to agency policies.
- Support youth’s educational stability by coordinating with schools and McKinney-Vento liaisons.
- Advocate for clients with employers, legal systems, mental health providers, housing agencies, and other community resources.
- Transporting youth to appointments, and other essential services as needed.
- Participate in staff meetings, house group meetings, case reviews, and ongoing training to enhance service delivery and coordinate care transitions.
- Perform other duties as assigned by the supervisor.
Qualifications
- Bachelor’s degree in social work, Psychology, or related behavioral science required.
- At least 2 years of experience providing direct services to at-risk youth and/or vulnerable families; residential experience preferred.
- Ability to effectively work with youth who are experiencing emotional and behavioral challenges, with demonstrated use of trauma-informed care.
- Demonstrated cultural humility when working with diverse populations, including street youth, pregnant/parenting teens, and LGBTQ youth.
- Strong written, verbal, and interpersonal communication skills, with the ability to collaborate with internal teams and community partners.
- Knowledge of community resources and ability to connect clients to services, including through outreach and cold calling.
- Proficiency in computer applications (Outlook, Word, Excel, and databases).
- Valid driver’s license and access to a personal vehicle for work-related travel.
- Flexible schedule to meet client needs, including on-call responsibilities (24/7 one week per month)
- Hours: 37.5 hours a week, some evenings as needed
