Job Summary:
We are seeking a highly skilled and experienced MDS Hybrid Position to join our team at Fletcher Rehabilitation and Healthcare Center. As a Hybrid MDS Coordinator, you will play a crucial role in the coordination and management of our nursing care plans, ensuring that our residents receive the highest level of care possible. This is a full-time position, with a blend of onsite and remote work (3 days onsite, 2 days remote).
Essential Functions:
- Keep abreast of current federal and state regulations, as well as professional standards.
- Assist with the development of comprehensive care plans for Residents in coordination with the MDS that accurately addresses the needs of the Resident.
- Coordinate, manage and monitor the written plan of care for each Resident of the facility that identifies the needs of the Resident and goals to be accomplished for each need.
- Monitor Resident status changes to ensure appropriate and timely nursing or clinical team involvement.
- Assure MDS and support documentation are accurate representations of the Resident and meet regulatory and auditor requirements.
- Perform regular audits of documentation to assure accuracy.
- Assist nursing management with the discharge process.
- Perform administrative requirements, such as completing necessary forms and reports.
- Assure that established infection control and standard precaution practices are maintained when providing care.
- Follow established safety precautions when performing tasks and using equipment and supplies.
- Maintain strict confidentiality regarding sensitive health information of Residents.
- Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons.
Required Education and Experience:
- Current State License as a Nurse
- C.P.R. Certified
Preferred Education and Experience:
- One year experience as a Nurse in a long-term care setting
