Responsibilities
- Provide comprehensive primary care in community-based settings
- Manage acute and chronic medical conditions
- Perform medication management and care coordination
- Complete Transitional Care Management (TCM), Chronic Care Management (CCM), and Medicare Annual Wellness Visits
- Collaborate with physicians, specialists, facility staff, and families
- Document care accurately in Athenahealth
Qualifications
- Washington State ARNP or PA license
- DEA license or ability to obtain
- Board Certified
- Experience in primary care, family medicine, internal medicine, geriatrics, or post-acute care preferred
- Strong clinical, communication, and organizational skills
- Valid Washington driver’s license and reliable transportation

Pay: $130,000.00 - $160,000.00 per year
