Responsibilities
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using appropriate patient transfer methods like Hoyer lifts and manual handling techniques.
- Monitor patients’ health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals promptly.
- Support medication administration and ensure compliance with care plans while adhering to HIPAA regulations to maintain patient confidentiality.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed.
- Provide transportation assistance for medical appointments, community outings, or errands to promote client independence and engagement.
- Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control standards are met.
- Document patient progress accurately in care records, communicate effectively with clients and team members, and participate in ongoing training related to dementia care, Alzheimer’s care, autism support, and other specialized needs.
Requirements
- Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments.
- Knowledge of patient observation techniques, vital signs monitoring, medication administration, and emergency response procedures such as CPR and first aid.
- Familiarity with care for individuals with dementia, Alzheimer’s disease, neurodevelopmental disorders like autism or intellectual disabilities support.
- Ability to perform manual handling tasks including heavy lifting and patient transfers using equipment like Hoyer lifts safely.
- Strong communication skills for effective client interaction and team collaboration; experience working with diverse populations including geriatrics and individuals with disabilities is preferred.
- Certification in CPR/First Aid is highly desirable; ongoing training in HIPAA compliance and patient personal care assistance is expected.
- Flexibility to work various shifts in dynamic environments that may include long-term care settings or community-based services.
