Job Summary
Alive & Well Healthcare is seeking a compassionate, experienced, and highly motivated Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) to provide mobile wound care services in patients homes, assisted living communities, skilled nursing facilities and residential settings throughout The Villages, Lake, Summter, Marion, and Seminole Counties.
The ideal candidate is confident working independently, passionate about improving patient outcomes, and committed to delivering high-quality, patient-centered care in the community setting.
The selected provider will initially manage approximately 15-20 established patients per week, with opportunities to grow as our practice continues to expand. Compensation is $75 per completed patient visit. Most visits average approximately one hour.
Responsibilities
- Perform comprehensive wound assessments and develop individualized treatment plans
- Provide wound care management including debridement, dressing selection, and infection prevention
- Monitor wound healing progress and modify treatment plans as needed
- Collaborate with physicians, home health agencies, caregivers, and interdisciplinary healthcare teams
- Educate patients and families on wound prevention and care management
- Maintain accurate and timely documentation in EMR systems
- Ensure compliance with all state, federal, and company regulations
- Travel throughout The Villages, Lake, Marion, and Sumter Counties to provide mobile patient care
Qualifications
- Current Florida APRN or Physician Assistant (PA) license in good standing.
- Minimum 1 year of wound care experience preferred . Home health or skilled nursing experience is highly valued.
- WCC Certification highly preferred. Candidates willing to obtain WCC certification will also be considered.
- Strong clinical assessment and critical thinking skills
- Excellent bedside manner and communication skills
- Ability to work independently and manage a mobile schedule efficiently
- Reliable transportation and valid Florida driver’s license
- EMR documentation experience preferred
- Experience in home health, geriatrics, or skilled nursing
- Knowledge of chronic wound management, diabetic ulcers, pressure injuries, and vascular wounds
- Familiarity with Medicare documentation and compliance standards
