Licensed Clinical Social Worker (LCSW)
This role involves providing in-home psychotherapy services to geriatric patients in residential and assisted living settings, with approximately 6–7 home visits per day.
Key Responsibilities:

Conduct in-home visits to provide psychotherapy to geriatric patients with a range of mental health needs.

Primary focus of this role is delivering counseling and emotional support, while also assessing psychosocial needs and connecting patients and their families with appropriate resources and services as needed.
Qualifications:
- Master’s degree in Social Work (MSW) from an accredited institution.
- Current state licensure as a Licensed Clinical Social Worker (LCSW).
- Experience in a mobile-home setting or with in-home care practices is highly preferred.
- Knowledge of CMS regulations related to documentation and billing.
- Strong communication and interpersonal skills, with a compassionate approach to patient care.
- Ability to work independently and manage time effectively in a field-based role.
