Responsibilities
- Deliver personalized nursing care to patients in their homes, including assessments, medication management, and treatment administration.
- Develop and implement individualized care plans based on thorough patient evaluations and medical histories.
- Monitor patient progress regularly, adjusting care strategies as needed to optimize health outcomes.
- Educate patients and their families on disease prevention, medication use, wound care, and lifestyle modifications to promote wellness and independence.
- Maintain accurate medical documentation using Electronic Health Records (EHR) systems while ensuring compliance with HIPAA privacy standards.
- Coordinate with physicians, therapists, and other healthcare professionals to deliver seamless multidisciplinary care.
- Respond promptly to emergencies or acute health changes by providing emergency care or triaging patients for further intervention.
Requirements
- Valid nursing license (RN) with current registration in the applicable state or region.
- Proven experience in home health nursing or acute care environments demonstrating strong clinical skills.
- Knowledge of health regulation policies, medical documentation standards, and confidentiality protocols such as HIPAA.
- Proficiency in medical software systems including EMR/EHR platforms for accurate recordkeeping and medical documentation.
- Certification in Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR), with First Aid certification preferred.
- Familiarity with physiology, anatomy, disease prevention strategies, and chronic disease management principles.
- Ability to perform triage effectively by assessing patient needs quickly and accurately under pressure.
- Strong understanding of medical terminology and physiology to communicate effectively with healthcare teams.
- Excellent organizational skills with the ability to handle confidential information responsibly while managing multiple patient cases efficiently.

Pay: $42.83 - $51.58 per hour
