Position Overview: We are seeking a full time compassionate, highly organized, and detail-oriented Medical Care Coordinator and Patient Care professional, to manage and streamline daily care for senior clients. In this role, you will serve as the primary bridge between elderly client, their families, and medical providers and various family tasks. You will be responsible for managing complex medical calendars, coordinating care plans, scheduling appointments, and ensuring our senior clients receive attentive, seamless care while also providing the care when needed.
Key Responsibilities
Care Coordination & Patient Advocacy
Care Plan Execution: Serve as the primary point of contact for senior clients, family members, and healthcare providers to ensure individual care plans are followed. Provide family with feedback and scheduling issues.
Provider Liaison: Communicate directly with primary care physicians, specialists, home health aides, and therapy providers to track patient progress and care updates.
Senior Advocacy: Monitor client wellbeing, ensuring their medical, physical, and emotional needs are being met attentively and respectfully.
Medication & Treatment Tracking: Help coordinate prescription refills, medical equipment deliveries, and follow-up directives provided by physicians.
- In Home Care: will help in providing in home health and care, including around the house services.
Appointment Scheduling & Calendar Management
Calendar Ownership: Maintain, manage, and update complex daily, weekly, and monthly calendars for appointments, therapy sessions, and caregiver visits.
Medical Scheduling: Schedule doctor visits, specialist consultations, lab work, diagnostic imaging, and non-emergency medical transportation.
Conflict Resolution: Proactively resolve scheduling conflicts and adjust calendar itineraries on short notice due to illness or emergency care changes.
Reminders & Confirmations: Send timely appointment reminders to client, family members, and caregivers, ensuring prep instructions (e.g., fasting before lab work) are clear. Also take and verbally communicate the various events with family.
Care Taker: Help with various tasks with family members.
Administrative & Logistics Support
Documentation & Records: Maintain accurate, up-to-date client files, medical histories, emergency contact info, and insurance records in compliance with privacy guidelines (HIPAA).
Transportation Coordination: Organize non-emergency medical transport or coordinate with family/caregivers to ensure seamless travel to and from appointments.
Family Communications: Provide regular, empathetic updates to authorized family members regarding scheduling changes, health developments, and upcoming appointments.
- In Home Care: help provide limited services for aged family members.
Qualifications & Skills
Experience: 2+ years of experience in medical care coordination, patient navigation, eldercare administration, or a similar medical office role.
Technical Proficiency: Hands-on experience with Electronic Health Record (EHR/EMR) systems, digital calendar platforms (Google Calendar, Outlook), and practice management software.
Elderly Care Knowledge: Familiarity with geriatric care needs, mobility challenges, Medicare/insurance protocols, and local senior care resources.
Communication Skills: Exceptional verbal and written communication with high emotional intelligence and patience for working with aging populations.
Organizational Skills: Outstanding time management, multitasking abilities, and meticulous attention to detail.
