Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications and injections accurately following prescribed protocols while maintaining detailed medical documentation in electronic health record (EHR) systems such as Epic or Cerner.
- Assist with complex procedures such as IV infusion, catheterization, tube feeding, and sterile dressing changes using aseptic techniques.
- Manage airway maintenance and ventilator support for patients requiring respiratory assistance.
- Conduct triage assessments to prioritize patient needs in urgent care or emergency settings like Level I Trauma Centers or emergency rooms.
- Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care strategies to optimize patient outcomes.
- Provide specialized care for individuals with dementia, Alzheimer’s disease, developmental disabilities, or those requiring memory or behavioral health support.
- Support infection control protocols and ensure compliance with HIPAA regulations to protect patient privacy and safety.
- Utilize advanced EMR/EHR systems such as Athenahealth or eClinicalWorks for accurate documentation of patient care activities.
- Engage in patient education on health coaching topics including nutrition, pain management, medication administration, and chronic disease management.
Experience
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Demonstrated experience working with diverse populations across multiple settings such as hospitals (including ICU and trauma centers), nursing homes, hospice care, or outpatient clinics.
- Critical care experience preferred, including ICU or Level II trauma center exposure.
- Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and medical documentation standards.
- Hands-on experience with IV insertion, phlebotomy, specimen collection/processing, and medication administration techniques.
- Prior work in geriatrics or long-term care environments such as assisted living or memory care units is highly valued.
- Knowledge of infection control practices and sterile processing procedures essential for maintaining a safe clinical environment.
