Job Summary
We are seeking a dedicated and highly skilled Primary Care Physician to provide comprehensive healthcare services within a community-based setting. The ideal candidate will possess a strong foundation in internal medicine, family medicine, or general practice, with a commitment to delivering patient-centered care. As a Primary Care Physician, you will be responsible for conducting thorough medical assessments, managing ongoing health needs, and coordinating care across various specialties to promote optimal health outcomes for diverse patient populations.
Responsibilities
- Conduct detailed medical history reviews and physical examinations to assess patient health status.
- Develop personalized care plans that encompass preventive services, chronic disease management, and acute care interventions.
- Utilize Electronic Health Record (EHR) systems such as Epic and EMR systems to document patient encounters accurately and efficiently.
- Perform diagnostic evaluations including the interpretation of medical imaging such as CT scans when necessary.
- Coordinate with specialists across disciplines including psychiatry, endocrinology, cardiology, dermatology, and others to ensure comprehensive patient care.
- Provide health coaching and education to empower patients in managing their health conditions effectively.
- Administer immunizations, perform basic procedures such as suturing, and manage medication administration following clinical guidelines.
- Ensure compliance with HIPAA regulations and maintain meticulous medical records in accordance with NCQA standards.
- Participate in clinical research or quality improvement initiatives aimed at enhancing patient outcomes and service delivery.
- Engage in discharge planning, post-acute care coordination, and follow-up to support continuity of care across inpatient and outpatient settings.
Experience
- Proven experience in outpatient primary care within hospital or clinic environments, ideally including Level I or Level II trauma center experience.
- Demonstrated proficiency with EMR systems such as Epic or similar platforms for documentation and patient management.
- Familiarity with a broad spectrum of medical specialties including psychiatry, cardiology, endocrinology, pulmonology, dermatology, and infectious disease care.
- Experience performing procedures such as suturing, IV infusion, airway management, and basic life support (BLS).
- Knowledge of medical coding (CPT and ICD coding) for accurate billing and documentation purposes.
- Background in managing complex cases involving acute pain management, post-operative care, hospice & palliative medicine, or critical care is advantageous.
- Strong understanding of physiology knowledge and clinical evaluation techniques essential for effective diagnosis and treatment planning.
- Prior experience working in inpatient settings or hospital medicine is preferred; familiarity with triage processes is beneficial.
- Commitment to ongoing professional development through participation in clinical trials or research projects related to primary care practices. Pay: $150.00 - $160.00 per hour
