We are currently seeking a Licensed Physical Therapist Assistant (PTA) to join our home health team. This role is ideal for a motivated clinician who values flexibility, enjoys meaningful one-on-one patient care, and thrives in an independent field-based environment.
Position Overview
As a Physical Therapist Assistant, you will work under the supervision of a licensed Physical Therapist to support patients in improving mobility, strength, balance, and functional independence within their home environment. You will play a key role in implementing treatment plans and contributing to positive patient outcomes through consistent, hands-on care.
Key Responsibilities
- Implement treatment plans developed by the supervising Physical Therapist
- Assist patients with therapeutic exercises, gait training, and transfer techniques
- Support mobility, balance, and strength-building activities
- Provide patient and caregiver education as directed by the care plan
- Observe, document, and report patient progress or changes in condition
- Maintain accurate and timely clinical documentation
- Communicate effectively with the supervising Physical Therapist and care team
- Follow all clinical guidelines, safety protocols, and home health regulations
Qualifications
- Active Physical Therapist Assistant (PTA) license in the State of Illinois
- Graduate of an accredited Physical Therapist Assistant program
- Current CPR certification
- Minimum of 1 year of clinical experience preferred
- Strong communication, organization, and time management skills
- Ability to work independently in a home health setting
- Valid driver’s license and reliable transportation within the Chicago area
Schedule & Compensation
- PRN / Per Diem position
- Flexible scheduling based on patient needs and availability
- $30 per hour
