For many patients and families, the Access Team is their first connection to our organization. The Hospice Access Team Leader helps ensure that referrals are handled promptly, communication is clear, and patients receive timely access to the care and support they need.
We're looking for a compassionate, organized professional who enjoys coordinating people, processes, and communication while supporting a team dedicated to serving others.
The Hospice Access Team Leader oversees the day-to-day coordination of the Access Team and collaborates closely with clinical, operational, and growth leaders to ensure patients receive timely access to hospice services.
This position is responsible for coordinating referral and admission workflows, supporting team performance, improving processes, and helping create a positive experience for patients, families, referral partners, and team members.
The ideal candidate is a strong communicator, effective problem-solver, and collaborative leader who thrives in a fast-paced healthcare environment.

Coordinate the daily activities and workflow of the Access Team.

Support referral intake, admission scheduling, and patient access processes.

Monitor team productivity, service standards, and response times.

Assist with staffing coordination and workload management.

Support, coach, and develop team members.

Collaborate with clinical, operational, and growth leaders to meet organizational goals.

Identify opportunities to improve efficiency, communication, and the patient experience.

Ensure accurate documentation and adherence to organizational standards.

Build positive relationships with patients, families, referral partners, and team members.

Monitor and report operational and performance metrics.

Minimum of three years of healthcare experience.

Previous leadership, supervisory, or team lead experience.

Strong communication, organizational, and problem-solving skills.

Ability to manage multiple priorities in a fast-paced environment.

Proficiency with technology and electronic medical record systems.

Bachelor's degree preferred.

Experience in hospice, home health, patient access, admissions, care coordination, healthcare operations, case management, or other healthcare-related leadership roles.

Experience leading teams and coordinating services within a healthcare setting.

Knowledge of hospice, home health, or post-acute care operations.
