Job Summary
We are seeking a dedicated Licensed Vocational Nurse (LVN) to join our Home Health team. In this vital role, you will deliver expert nursing care directly in patients’ homes, ensuring their safety, comfort, and well-being. Your expertise will help clients maintain independence and improve their quality of life through comprehensive assessments and personalized care plans.
Duties
- Perform detailed patient assessments, including vital signs, physical examinations, and medication reconciliation.
- Administer medications, injections, IV infusions, and other treatments following physician orders and safety protocols.
- Manage wound care, catheterizations, tube feedings, and ventilator support as needed.
- Educate patients and caregivers on health management, medication administration, and disease processes
- Document all patient interactions accurately
- Collaborate with interdisciplinary teams to develop and update individualized care plans aligned with long-term care regulations.
- Ensure compliance with HIPAA confidentiality policies and infection control standards during all patient interactions.
Experience
- Valid LVN license with State of CA
- Experience working with individuals with developmental disabilities or in hospice care environments is highly valued.
- Knowledge of ICD coding (ICD-9/ICD-10), medical terminology, and medical documentation standards essential.
- Prior experience in home health, skilled nursing facilities, or long-term care settings is a plus.
- Strong communication skills for effective caregiver-client interaction and team collaboration.

Pay: $38.00 - $42.00 per hour
