As we continue to grow, we are seeking an experienced Hospice RN Case Manager who values quality patient care, strong teamwork, and the opportunity to make a lasting difference in the lives of patients and families.
Your Role
As an RN Case Manager, you will coordinate and oversee hospice care for patients residing in private homes, assisted living communities, memory care communities, and skilled nursing facilities throughout our service area.
Key Responsibilities
- Complete admissions, comprehensive assessments, recertifications, and routine nursing visits
- Develop, implement, and manage individualized plans of care
- Coordinate care with physicians, facilities, families, hospice aides, social workers, chaplains, and other interdisciplinary team members
- Provide education, guidance, and support to patients and caregivers
- Identify changes in condition and implement appropriate interventions
- Maintain timely documentation and communication
- Advocate for comfort, dignity, and quality of life for every patient

What You Bring
- Active Illinois RN License
- Hospice experience required
- Strong assessment, documentation, and care coordination skills
- Ability to work independently while remaining engaged with the interdisciplinary team
- Reliable transportation and willingness to travel throughout our service area
- Compassion, professionalism, and a commitment to exceptional end-of-life care
